Signature module in forms

Add to form A signature field for the patient can be added to forms and questionnaires. Go via [Settings] > [EMR] to [Forms library]. Here you can create a new form or modify an existing one. Additional explanation about this can be found in this article: Creating Forms


Drag the signature field into the form and set the desired settings:
  1. Label: Title that is displayed
  2. Footer Label: Text that is displayed below the field
  3. Background Color: The background color of the field
  4. Pen Color: Color of the signature
  5. Description: Text that can be displayed below the signature field
  6. Hide Label: Check this box if you do not want to display a title
  7. Validation: Check this box if this is a mandatory field

Use

The sent questionnaire can now be signed by the patient via their computer or tablet/smartphone. More information about sending questionnaires can be found here: Sharing a form with a patiënt
